I'm using Premier 6.0 and when I go to add a simple cross-fade between clips and preview, it never seems to work right. I've had everything from it just showing a black picture with the codec name on bottom (I'm using a unregistered version of PICVideo and will change it to the original source later), it flashing another random clip, to just locking the first frame of the original clip.
Ideas?

Don't use those crossfade effects. They suck.
See the little arrow under "Video track 2"? Click it.
Now see the red lines in the new yellow space(in your vid clips)?
Well, those are called rubber bands. Put whatever you want to fade in or out ontop of your last clip.
Click the red line and drag it so one end is at the bottom, and the other end(of the clip) is at the top. When a rubber band is going up, that clip is fading in. When it's going down, it's fading out.
Mess with these until you find the best way to do it. =p

No it's not. Just do a 2-4 frame white picture and fade it out with a rubber band. =\

Well, there are some transitions that have use. To answer your question, you may have set up that the transition both begin and end on the same layer of video, so nothing will happen.
Right-click the transition layer and select Transition Settings. There, it will give you a demonstration of the transition using the letters A and B (you can select a box to use your sources). You can start the transition and end it anywhere from 0% to 100%. Also, you can switch which layer is transitioning to the other. In the lower right corner of the Transition Settings box, there's a little demo of the transition. To the left, there's a blue arrow pointing either up or down. Click it to reverse the the direction of the transition. This arrow is also on the transition on the timeline.

Are you rendering preview files?
