Pick one - click it and the download should start. If one of them doesn't download automatically, click the next - just go through them till you find one that works (the first probably *will* work - it doesn't matter which one you click most of the time). Once the file is downloaded, open it and it will automatically install the codecs. Personally, I prefer in/direct to local, mostly because I don't like having to give out stars, largely because I'm very lazy when it comes to actually watching the videos I've downloaded (and just in general as well).
That's one reason I always try to provide direct as well as local links for own videos... that, and the fact that MPEG-1 is just generally more compatible than DivX-FourCC'ed XviD.

Usually, if i can use bandwidth other than the one people donate for, I do it, so that the maximum bandwidth remains available for people who download videos that are available in local only, and more generally for people browsing the site (it's getting quite slow at times, but I guess it's more a question of database server load than a question of bandwidth).
Of course, if I notice I get 10 kB/s with (in)direct, I quickly switch to local...
