I don't know if it has a formal name, but the nickname I know it by is "Piano Keys." You can do it using the Four-Point Garbage Matte on each clip of source footage (under Video Effects -> Keying).

Personally i would just mask out a rectangular shape and add the effect to every layer, then change the position of the mask by rotating it and/or altering the X and Y values. This is more basically done in After Effects although you can do the same job with Premiere pro
