- Slayers (but sometimes it might be repeatable due to spell casting methods)
- Ergo Proxy (not THAT much action, but when it happens, well, they have skills)
- Keroro Gunsou (maybe, thinking of repeatable action scenes, none comes in my mind because Keroro is a fool, but a quite creative character; but since there are so many episodes, it might repeat sometimes....)

And he, how about Street Fighter? I didn't see that much of it, but I remember it was quite fine in terms of action.

It's a movie, rather than a series, but Sword of the Stranger has some great action scenes.

Katekyo Hitman Reborn! ....'nuff said. It does suffer from some of the explaination stuff, but most shounen anime do. At least it has interesting fight scenes and doesn't focus solely on the main character.

A lot of anime films and good OVAs avoid re-using scenes. First one that comes to mind for me is that Fist of the North Star movie. The TV series, on the other hand, had plenty of reused scenes.
Cyber City Oedo 808 had some good fights, in my opinion.
Other than all the photon beam spamming, Mazinkaiser had relatively "fresh" animation.
And who could forget the Ninja Scroll OVA?
