Man there are so many that can be good, but there are a few that tops the lists in my opinion.

Naruto: 3rd Season Opening and Fighting Dreamers Opening
Bleach
Hellsing
Case Closed (not Adult Swim opening but DVD/actual show openings) First the opening with Mune do Doki Doki in English is my fave as well as the one with the song Nazo(English version) for the opening.
Madlax
Zoids Chaotic Century/Guardian Force Opening

And for endings/credits:
Zoids New Century
Zoids CC and GF
Bleach
Tenchi Muyo
Gundam Wing

and...

well I can't think of any others right off the top of my head now. But that about does it I think.

Right now one of the most clever openings I have seen is the one for Excel Saga. It had me and my brother doing double takes and saying "WTF" for days....

The intro for Episode 26 of Excel Saga.

The Intro for Puni Puni Poemi is TEH BEST though. The lyrics are classic and the sheer irony of the visuals is just as great.
