It's Kiyone from Tenchi Muyo, and she's, of course, talking about Mihoshi. I don't know what series it is, except it's not the OVA.

That'll work!! And for the record, it was the first TV series, aka Tenchi Universe.

And, of course, Mihoshi isn't dead. She's just taking a nap.
