Subtitles... they are annoying.
Quality... ehh I'm not particularly picky, but if it's really bad, then blech.
Main turn-off(esque?), you ask? Videos that are too general! I want to watch a video that tells a story within itself, not one that pastes the opening of the first episode with different music! I find alot of music videos are too broad, and use whole scenes, one after another without any real connection. If I see that the mood and the timing is in sync, its really attractive, and I'll keep watching.
Also, originality, or lack there of. Please, don't make a Princess Tutu amv to the tune of "Don't Stop the Music", or "Everytime We Touch." Actually, Everytime We Touch is so old, then just back faaaaaaaar away from it. Or I will be forced to not watch it and/or shoot you :] with a marshmallow gun, and then eat said marshmallows in your face! BWHAHAHAHA----- Where was I? oh, yes....
8 minutes.... please no. 3-5 is a-ok! ;D Also, effects are cool, good for transitions, but don't overdo the pixely ones everytime your changing a scene, thats lame! =]
Once again, I'm agreeing 90% (kidding, 100% ^^) with DriftRoot. I consider an excellent amv when it has (as stated before) a, well... plot! No one really wants an amv with a second-rate overused song jumping all over the place (Even if it's about Naruto and his friends, uhh he has like a million.... too broad) That's a definate turnoff!! Instead, use a catchy (note i'm not saying anything about speed, tempo, or mood, because different animes have differing compatable songs, but in Naruto's occasion, maybe a fast paced song, etc...) ....errr anyway. Use a catchy song that meshs well with Naruto's relationship and effects of the kyuubi... use your imagination!

I think a lot of folks have already mentioned many of my own AMV pet peeves, but I figure I'll add a couple more.
First, I really find it grating when an AMV is so overly processed and has so many effects added that it's too perfect-looking. If I am watching an AMV, but I can't tell that there was a human being behind the scenes making everything, it really loses some of the enjoyment factor. It feels like there's no heart to it when everything is so gorgeous and shiny. I personally find the teeny little nitpick flaws really endearing, as it shows someone put time and effort into making it, instead of the vid looking like it was stamped out by some kinda robot.
Another weird thing I see periodically are vids that are edited together in such a way as to tell some sort of story. F'rinstance, I saw one FF vid where it was the continuation of a previous one. Rinoa had cheated on Squall with the villain from X-2, and Squall goes through all sorts of hell, blahblahblah. It was so strange, I didn't even give it my typical 30 seconds.

And this is why I primarily use rock music when making Princess Tutu AMVs. Seriously, think outside the box, folks. Not only on the music, but also on the subject matter. I see so many videos mainly on YouTube) that focus on Duck, or Duck and Mytho, or whatever other popular pairing du jour fans latch onto. Never mind that there's so many other characters that could use AMV love.
