I think that should have been in spoiler tags (I say that as I had read your post before I had a chance to see the episode).

Anyhow, this episode really cements in my mind what I'd been thinking for a while now; that this is the best show this season.

By how the episode ended & Misaki's reaction, I'm kind of wondering if Tokako was who Misaki thought was dead. Furthermore, I was kind of thinking that since there were two extra students, could it be that there are two dead students as well?

Maybe the lady had some relationship with one of the previous no. 3 classes/was witness to the previous events and meeting the current class was just too much so she snapped.

Well it could have been revealed near the end of the episode and then the last episode would be trying to knock off the dead person.
