The information I provided in the other box doesn't show up anywhere in this confirmation email, so I wonder where it even went.

I realize this is all my fault because you clearly say "One souce per line, please use the format: artist - title," but why is one box labeled "Artist(s)" and the other labeled "Source Audio"?

I looked into this.
The wording on the form was supposed to be different, not sure how that happened. Enter the data to the best of your ability. If there is question I'll reach out to you individually.
