Anime Trivia [Please read 1st post before posting questions]

Okay, this trivia question comes from the anime -- WAIT, I can't say, as it's part of the answer.

For an intriguing piece of anime music, take this following recipe:

-- Take the fourth movement, the "Allegro con brio," from the 7th Symphony by Ludvig von Beethoven as a base. (Do NOT forget to give credit to the original composer)

-- Season to taste, using pieces from other works of von Beethoven. Suggestions: season w/ parts from Beethoven's 5th and 9th Symphonies. (others MAY be in there as well)

-- Develop your own wacky/silly lyrics, which have nothing to do with the OAV series (or Beethoven, for that matter). Have the seiyuu for the main character perform said lyrics.

-- Serve during the ED sequence.

The question: Provide 1) the name of the anime series that uses the above recipe, 2) the name of the song, AND 3) the name of the seiyuu who sings these lyrics -- very very FAST!

Song - My Omelette (Watashi no Tamagoyaki)
Seiyuu - Kotono Mitsuishi

i think im right........ not overly sure about the seiyuu

an odd anime, its a very abstract piece of art........ if you havent seen it, your best bet will be to look it up..........
