Otohiko's basically right when he says that prog metal isn't really a genre. However, that doesn't stop certain bands (such as Dream Theater, Fates Warning, Hourglass, Andromeda, Pain of Salvation, even Symphony X, and several others) from being classified as such. In cases such as those, the similiarity seems to lie mostly in guitar/keyboards and often subject matter. Of those bands listed above, SyX is the one that stands out in subject matter (focusing more on fantasy than the other bands) but many still classify them as prog metal because of the style of the guitar/keyboard solos and changing time signatures. Some other things you'll often find in much prog metal are classical-influenced riffs and tonal patterns, purely technical drums, and slightly higher pitched vocals (falsetto or vibrato).

There's a lot of ways the genre/style/category system can be twisted and bent, since rock bands don't tend to go very well into standard taxonomic pigeonholes. And the more progressive you go, the less standardised it gets. Quite simple.

As for prog rock and prog metal, there's similarities and trends between bands not because of the sheer progressiveness, but because historically, prog bands focused on certain themes, instruments and techniques. So, these stylistic signatures sort of stuck. Still, if something complex and unclassifyable comes along, it's probably prog.
