AviSynth-->Mjpeg
Mjpeg-->Premiere
Premiere-->CorePNG\Huffy or whatever
CorePNG-->AviSynth
AviSynth-->VirtualDubMod
VirtualDubMod-->CorePNG
CorePNG-->ColorCorrection
ColorCorrection-->CorePNG(or an AVS for a last touchs)
CorePNG-->VirtualDub(for an xvid\mp3 version)\TMPGEnc(for a "master" copy Mpeg2\Wav)

and for the audio:

... ok - here is how NOT to do it:
DVD --- > SmartRipper --- > VOB to hard drive.
VOB --- > VDmod 1.5.4.1
Filters Video:
Deinterlace ( Blend )
Spatial v1.0 ( 5, 6 )
Dotcrawl ( 3, 4 )
Resize 1440 X 1260 ( Precise BiCubic = 1.00 )
Auto-Focus v2.3 ( line desired = 2, Smire Colors = 23 )
Resize 640 X 480 ( BiLinear )
--- > CorePNG/Huffy 2.0

Edit

New Huffy 2.0 --- > VirtualDub 1.4.10
Filters Video:
Resize 576 X 432 ( BiLinear )
Temporal ( 1 )
Filters Audio:
MP3 48k sampling at 224bit rate.
settings: Xvid ( 1234 constant bit rate )
--- > whatever.avi

This is not recommended for anyone but a Noob - an even then, you'll start
to understand "why" when you see the final AVI ~_~

notes:
both versions of VirtualDub are on-line.
all the Filters are available on-line.
all Codec's are available on-line.
A multi-lined video editing system, see the Guides, are available.

Please take a very-very hard look at the finished AMV before desiding how
great this process doesn't work. ( yes, it works - but is it right for you? )

The easiest way to convert AVI to MPEG is to use TMPGEnc.

Why bother removie the Audio before going into WMM? why not simply mute the audio to the clip, IN WMM ?

Strange, I've always ripped mine as AVIs or MPGs straight from the dvd VOBs, not from VOB files on my pc, and they've turned out pretty good quality. Takes a while to encode everything, but I've found that it's usually worth it.

Here's what I do:
* Load DVD VOB files with DVD-DIVX. (Strange program, but reasonably easy to use and gives decent results once you get used to it.)
* Rip with DivX 5.05 or XviD, no sound. Any screensize that's combatible with the encoder is available.
* (If using Premiere, I dunno about WMM) Convert files to .MPGs with TMPGEnc.
* Load clips into the video maker and start editing.
* Once finished, export as an AVI file. (WMV if using WMM)
* (If using WMM) Load the WMV in "Stoik Video Convertor" and then convert to an AVI.

I dunno if it's a good way to do things, but I've always done stuff that way. *shrug* I'm probably doing it the wrong way and I found a load of errors with interlacing when I first ripped files, but now they're fixed. Otherwise it's an easy way to do things.
I used to use VirtualDub, but now that's just for screenshots or whatever.

Oh gosh. TWO recompressions even before being sent to the video editor? That can't be good for quality.

I'm forced to recommend the ripper --> indexer --> AVISynth method that Otohiko mentioned (well, at least he linked to an explanation).
