I remember seeing Iggy Pop in a movie somewhere, and then I think playing Ellen's dad on the old Nickelodeon show "The Adventures of Pete and Pete" or some such role. From cutting yourself onstage with broken glass to children's TV acting is quite the leap.
And who was it, Jason Schwartzman? who is also in Phantom Planet. I think it was him.
Anyone care for Adam Sandler's all-musical album, "What's Your Name?"
"Weird Al" Yankovic had his own movie ("UHF" for the uninformed).
Mos Def was in the Hitchhiker's Guide to the Galaxy remake.
Ice Cube actually had a really solid acting turn in Boyz N tha Hood.
Keanu Reeves plays bass in Dogstar (unless that doesn't count, since it's not really 'singing').
And where do we classify Tenacious D?
And oh dear, I just thought of all those J-Pop idols with voice acting work. That might complicate things.

I'm not as offended by the Shatner/Nimoy ventures, mostly because they're funny as hell and are good for making really weird AMVs......
I think we forgot the Bad-Actor-Turned-Bad-Singer (Juliette Lewis, what ARE you doing?!) and Bad-Singer-Turned-Bad-Actor categories (Mark Wahlberg).
I'm not exactly sure what you're trying to do with this thread. Just make a list? If not, I don't see there being much room for actual discussion in this thread, because there's no way to determine who is or isn't a good actor/singer, it's all opinion. So, really, there's no way to make a list either because not everyone will agree that so-and-so is a good actor/singer.

Um, I can't think of anyone else at the moment for all four categories.
The late Telly Savalas decided, after his Kojak stardom, that he could sing. Although he had a great voice, this did not extend to the ability to carry a tune, so he had to confine himself to mostly speaking in the songs he covered. Herve "Tattoo" Villechaize put out an original song where he wonders "Why doo people hef too fight? Why can't they learn to doo what's ghrrright?". Uri Geller isn't an actor, but he tried to sing as well.
While it probably doesn't count per se, some people took Vincent Price's cooking albums and did the ol' cut and paste routine so the legendary horror star could posthumously continue the rap legacy he initiated by appearing in Michael Jackson's song "Thriller". Those mp3s are a lot like the Dan Rather "song" "Rocked by Rape" that Scintilla made an AMV to. I think they may even been made by the same people. Some (presumably other) people did something similar to a JFK speech ("Let us begin anew") but that wasn't a rap song.
Howard Stern has sung in a number of songs, but he isn't principly an actor, and those songs are for the most part parodies.
Tom Waits (sp?) has appeared in a number of Francis Ford Coppola films, most notably Bram Stoker's Dracula. I'm surprised nobody has mentioned Prince.
