The best codecs are called "Uncompressed RGB" and "Uncompressed PCM", both developed by The Uncompressed Codec Group. I'd link to them, but their website is out for server maitenance. Read THIS and THIS instead.

XviD and MP3. Technically x264 is a better video codec, but it's quite a bit more complex than XviD, from my experience at least.

WRONG! ^_^

Once again, I repeat, On2 VP6 or VP7. It even supports alpha transparent video streams. Only downside is that it kinda limits yer choices on audio codecs, pretty much stuck with teh craptastic MP3 codec (there are FAR better alternatives). Actually though, it ~might~ be possible to produce non-standard FLVs using other audio codecs...but then they won't be so Flash friendly.
