I dont think that they changed the ending too much...it was pretty similar to the manga ending, even if they kind of fast fowarded the timeline several years into the future. I just wish they had done a whole arc on the fights and stuff instead of two 30-45 min episodes. All in all I thought it was a fitting end to a series, even if it was a bit abrupt.
Someone already corrected me on the OVA...if you are still referring to the *second* OVA like I am (Reflections in the US), it was *very* incorrect in the ending. First off, the manga for the "Revenge Arc" was 10 volumes this is roughly equivalent to 40 episodes. Obviously 2 OVA episodes is not this and cannot sum up even 5% of it.

Secondly, have you even read the manga? The ending in volume 28 goes about 8 - 10 years into the future and Kenshin and Kaoru are married and have a baby named Kenji. Yahiko has grown into a respectable man and fights Kenshin...Kenshin gives him the sword (yeah i know some of this is in the OVA briefly).
At the end, Yahiko runs off into the horizon and Kaoru and Kenshin are standing together with their baby, happy, and they notice his scar is fading. That's it! That was the ending. It was happy and open-ended. There was no stupid war Kenshin went to or some disease he got. And most importantly he and Kaoru did not die.
So again, did you read the manga? Because the ending is QUITE different. I don't even consider the 2nd OVA as part of the true timeline since well...it's not.
