For me the most graphic would have to be 3x3 Eyes.
Extremly gory and a grate story line but I dought you've heard of it

As for sexual material, Bible Black sweeps the floor. I've never seen anything so cut-loose and sadistic. And I'm only on episode #2!!! I'll bet it's illegal in at least 50 countries!!! But then I can't help but continue. I have to see what happens? Don't you hate it when that happens?

I leeched Bible Black, have not seen it though,
Most graphic violence wise was most likely opening scene to the Samuria X ovas,
that's just nasty. My mother was like OMG WHAT IS THIS!!
That Ukarifixkaskmsaklmdskal (something arrather) Return of the OverFiend and what not, that was voilence and Henti combo.. totally twisted...

I've only seen one 3X3 Eyes episode and didn't really care for it. Where Yakumo chops off his own forearm and then asks everybody where it is when the episode is nearly over. But it was more funny than violent since he's immortal and can't die anyways, and he'll probably have a brand new arm by the next episode.

I'd have to say Genocyber ties with Akira for goriest scenes in an anime for me. There's this one segment that just seems put into Genocyber specifically to gross you out. It starts with a nurse sitting on a bench in a dark hall. It doesn't seem like anything weird at first, but then it cuts to a view of the dark hall that a detective is walking down. Then it shows the nurse again, although this time her head slowly rips off: there is a soft ripping sound and you watch the tendon strands connecting her head to her neck stretch taught and break. Switch back to the scene of the hall, now littered with body parts, mostly severed limbs and organs, with the detective's footfalls grow louder and more panicked. Burst through a set of doors to see these weird scientist garbed humanoid creatures with faces like the tree spirits in Princess Mononoke holding scalpels and crouched over a slew of mutilated bodies. All with organs spilling out and blood pooled all around. Then the detective seems to wake up from that nightmare, and notices his glasses are shattered. He looks down at himself and finds that he's being dissected... while alive: The skin over his ribcage is pulled open and held by tiny pins with his stomach, liver, intestines, and other organs completely exposed. The segment ends with him giving a horrified scream.

Nothing is grosser than watching a body getting dissected, seeing it animated is only slightly less gross.

Ninja Ressurection was the most tasteless. I hated it. They had a guy get his goddam arms choped off, then get stabed through the chest, and then have sex AS HE WAS DYING with a girl who'd just been MOLESTED BY HER SATANIC FATHER!!!!!!!

And THEN she gave birth to a fully grown man who EXPLODED out of her. What the hell is wrong with these people?!

And what I saw of Bible Black (the first 7 min of the 1st ep) wasn't that graphic, just weird (and kinky oO).
